To Order a Report:
Online credit card payments are securely processed through PayPal. You do not need to have a PayPal account to use its payment processing system. Your PDF will be e-mailed to you within one business day after placing the order. If you use e-Check as your payment service, please be advised the PDF will be sent to you upon confirmation that payment has been received, which may take up to 5 business days. If you have any questions or concerns, please contact us.
Purchase orders are accepted from higher-education institutions. Downloaded forms may be faxed to (518) 207-0089; e-mailed to admin@checs.org; or mailed to CHECS, P.O. Box 38083, Albany, NY 12203. Download a Purchase order now.
Site Licenses are available for PDF reports for $100 per license. The license grants an institution the right to give access to the PDF to all of its current employees. When you order a site license, you will receive a PDF version of the report as well as a site license for that report.
Printed Copies are available only if noted and are mailed via U.S.P.S.
PDF Copies are available for all reports. Once an order is placed, your PDF will be sent to the e-mail address you've provided within one business day. All PDF copies are password protected.
